Agency transition overview

On May 23, 2023, Gov. Tim Walz signed into law two pieces of legislation that will bring big changes to the Department of Human Services, paving the way for the creation of a new Department of Children, Youth, and Families (DCYF) by July 1, 2024, and establishing the Department of Direct Care and Treatment (DCT) as a standalone agency in 2025.

DHS will mail the form only when requested by Medical Assistance and MinnesotaCare enrollees. We will not automatically send the form out. Follow the instructions on this webpage to request your form 1095-B.
