http://www.careproviders.org/ features information to help individuals and families determine the need for and the most suitable long-term care option for themselves or an older family member. Includes list of agencies and websites at the federal, national state and local level. Also includes a series of national fact sheets with questions and web resources regarding the process of determining the need for long-term care through finding suitable options with a facility finder feature also available.

Honoring Choices Minnesota

www.honoringchoice.org - This statewide advance care planning initiative is a unique partnership between health systems, insurers, providers, and the community at large. Honoring Choices Minnesota's aim is to transform conversations about health care wishes for all Minnesota adults ages 18 and over. A free downloadable health care directive in English, Spanish, Somali, Hmong, and Russian is available on the website. More than 600 video-taped conversations on beginning advance care planning conversations and practical resources for all Minnesotans are highlighted. Who would speak for you if you couldn't speak for yourself? Learn how to begin your own family-based advance care planning conversation.
Minnesota AARP

http://www.aarp.org/states/mn provides many resources on retirement and long-term care planning. Special program underway called Decide. Create. Share that focuses on special needs of women in planning for their retirement and long-term care.
Minnesota Board on Aging

www.mnaging.org - The Minnesota Board on Aging is a governor appointed board that is a cosponsor of this effort. The Board designates and funds a network of seven Area Agencies on Aging that provide information and assistance, fund supportive services in their communities and advocate for the needs of older persons in their local areas.
Minnesota Department of Commerce

http://mn.gov/commerce/ provides a wealth of information for consumers about long-term care-related insurance and financial products on its website. The department also has a telephone number to call to talk to an expert on insurance and related products. Contact the Minnesota Department of Commerce Consumer Response Team 651-296-2488 OR 1-800-657-3602 (outside Metro area) or email Consumer.protection@state.mn.us for information on insurance in Minnesota.

MinnesotaHelp.info is an objective State of Minnesota website that lists over 20,000 different agencies and organizations for all ages.

The award winning Long Term Care Choices Navigator is available to help people successfully remain in their homes or community. It is searchable by key word, zip code, service or issue. This database includes all home care agencies, senior housing, health plans, nursing facilities in the state.

Senior LinkAge Line, you can find many services that provide some type of aging or long-term care service including Long Term Care Option Counseling that can help you remain in your home. Call 1-800-333-2433.

Senior LinkAge Line One Stop Shop for Minnesota Seniors is operated at the state level by the Minnesota Board on Aging and in all areas of the state by the Area Agencies on Aging. The Senior LinkAge Line is also designated by Medicare as the State Health Insurance Assistance Program for Minnesota. Senior LinkAge Line is a partnership of the Minnesota Board on Aging and its designated Area Agencies on Aging serving all regions of the state.

Disability Hub MN is operated at the state level by the Disability Services Division within the Department of Human Services with five call centers throughout the state, including two Independent Living Centers. Call 1-866-333-2466 or visit www.disabilityhubmn.org.

Veterans Linkage Line is operated at the state level by the Department of Veterans Affairs with links for other agencies serving vets including the county veteran service offices. Call 1-888-546-5838.

If you access the One Stop Shop for Minnesota for Long Term Care Options Counseling through the Disability or Veteran's linkage lines, you will be transferred to the Senior LinkAge Line.
