Honoring Choices Minnesota is an initiative of the Twin Cities Medical Society and the East Metro Medical Society Foundation to encourage families and communities to have discussions regarding end-of-life care choices.

The Honoring Choices website contains a free, downloadable health care directive in five languages: English, Hmong, Somali, Spanish and Russian.
Twin Cities Public Television supports Honoring Choices Minnesota with efforts to reach and engage Minnesota citizens in this effort. An active partnership with Twin Cities Public Television has produced more than 600 video conversations, a documentary series and practical resources to reach and engage Minnesota citizens in this statewide effort.
