Behavioral health home services

BHH services will follow these principles:

• Use a multi-disciplinary team that shares information and collaborates to deliver a holistic, coordinated plan of care.
• Address the individual’s physical, mental, substance use and wellness goals to better meet the needs of individuals experiencing serious mental illness and their families.
• Take a person-centered approach, and engage and respect individuals and families in their health care, recovery and resiliency.
• Respect, assess and use the cultural values, strengths, languages, and practices of individuals and families in supporting an individual’s health goals.

The Minnesota Department of Human Services (DHS) must certify providers to deliver BHH services to qualifying people enrolled in MA.

Some people enrolled in MA may be eligible for BHH services. Find information about BHH services for individuals and their families, including eligibility for the services.

Partnership with NAMI-MN

To develop the BHH services model, DHS entered into an agreement with the National Alliance on Mental Illness Minnesota (NAMI-MN) to gather feedback from people enrolled in MA living with mental illness across the state. Focus group participants concentrated on these topics:

• Access to physical and mental health care
• Transition of care experiences
• Methods of obtaining health information
• Opinions about the facets of integrated care
• Inclusion of individual, cultural, spiritual and gender values into the care process

DHS is using the feedback from these focus groups to shape behavioral health home policy. The full report of Behavioral Health Home Recommendations is available on NAMI-MN's website.

BHH services evaluation reports by Wilder Research

In September 2017, Minnesota contracted with Wilder Research to evaluate the implementation of Behavioral Health Homes (BHH) services. The evaluation included: (1) individual interviews conducted in partnership with NAMI Minnesota to learn how participants in this integrated team-based model of care have changed their beliefs and behaviors; (2) staff interviews with BHH services providers about the implementation of BHH services, as well as their referral processes and progress in developing care coordination services and identifying needed supports; and (3) analysis of state cost and quality data.
