2025 Care Coordination Conference Information

The Minnesota Department of Human Services (DHS), along with managed care organizations, are hosting a virtual Care Coordination Learning Day Conference. This conference is being held on the morning of November 4 and the afternoon of November 6, 2025. This conference contains the following learning sessions: 

• Assistive Technology by Alaina Gallagher from Association of Residential Resources in Minnesota (ARRM)
• Self- Care and Mindfulness by National Alliance on Mental Illness (NAMI)
• Care Coordination 101 by Managed Care Organizations
• Guardianship and Power of Attorney by Marit Peterson of Minnesota Elder Justice Center
• Community First Services and Supports (CFSS) by DHS CFSS Staff 
• Housing in Minnesota by Gina Smith and Housing Team from DHS
• Unconscious Bias by DHS’ Office of Equity and Inclusion
