Feb. 12, 2026: DHS corrected 268 ratings on the Assisted Living Report Card.
On Feb. 12, 2026, DHS corrected some of the resident health, safety and staffing ratings for 268 assisted living settings. The corrections were made after DHS found errors in ratings during a thorough quality review of resident health, safety, and staffing ratings published to date. The data used for these ratings came from the Minnesota Department of Health (MDH) and were used by the University of Minnesota (UMN) to calculate ratings for the Assisted Living Report Card. After identifying the errors, MDH, UMN and DHS reviewed the data, calculation methods and reporting process together. The corrections are due to data and calculation issues and are not related to provider performance.
The review found several causes for these errors, including data entries that incorrectly showed surveys that did not take place and mistakes in how calculations were applied across different datasets and survey years. Although DHS discovered these issues after the ratings were already published, it is responsible for ensuring that publicly reported ratings are accurate and reliable. DHS made these corrections to ensure ratings reflect the most accurate available data.
DHS, MDH, and UMN are committed to strengthening their data review and quality checks to help prevent similar issues in the future. These steps include improving documentation of data sources and analytic decisions, stronger methods to reproduce ratings calculations, and implementing additional quality review steps. These improvements reflect a shared commitment to transparency, accuracy, and continuous improvement in public reporting.
June 9, 2025: DHS published the 2025-2026 Minnesota Department of Health (MDH) survey ratings thresholds.
- DHS will use thresholds based on 2023 and 2024 MDH assisted living licensing survey results to assign the next round of resident health, safety, and satisfaction ratings. To view these thresholds, go to MDH Survey Ratings Thresholds and Peer Groups (PDF).

March 20, 2025: DHS published the 2025 quality of life survey ratings thresholds.
- DHS will use thresholds based on 2024 quality of life survey results to assign the next round of resident quality of life and family satisfaction ratings. To view these thresholds, go to Quality of Life Survey Ratings Thresholds: 2025 (PDF) .
- Vital Research, under contract with DHS, plans to begin the 2025 round of quality of life surveys in April 2025. DHS plans to publish the next round of resident quality of life and family satisfaction report card ratings in early 2026.
June 3, 2024: Minnesota Department of Health (MDH) surveys completed on or after July 1, 2024 will be used for new Assisted Living (AL) Report Card ratings.
- DHS will add new quality ratings based on MDH surveys to its AL Report Card in early 2025.

- Minnesota assisted living laws require MDH to survey licensed assisted living providers at least once every two years. The purpose of these surveys is to evaluate and monitor the care and services provided to assisted living residents and determine if the provider is complying with licensing requirements.
