At the Minnesota Department of Human Services, we work to help the people we serve obtain and maintain appropriate housing by supporting systems that bring together housing, services and income supports to enable people to live in the community of their choice.

People at risk of homelessness

The PATH program helps people with mental illness or substance use needs who are homeless or may become homeless soon. It connects them to outreach, case management, housing help and local services that support recovery and stability.
