You can submit your completed renewal forms in one of several ways to your county, Tribal or state agency.
ONLINE: When you’re ready to upload your documents, choose the submission path based on your household eligibility and follow the instructions provided.
- If you have MA or your household has both MA and MinnesotaCare, upload documents to MNbenefits.
- If your household only has MinnesotaCare, upload documents to DHS Health Care Consumer Support Document Portal for Consumers.
IN PERSON OR FAX: Drop off the documents or fax them to the agency listed as the return address on your paperwork. Addresses and fax numbers for agencies are listed in Minnesota Health Care Programs county, Tribal and state directory.
MAIL: Put documents in the return envelope provided, address the envelope using the agency address in the upper left hand corner of the notice, add postage, and put it in the mail. If you don’t have the first page of your renewal notice, see the Minnesota Health Care Programs county, Tribal and state directory.
PHONE: Give your information verbally over the phone. Contact your county, Tribal or state agency to learn more. Note: Phone renewals may take up to an hour or longer.
