DHS Licensing Information Lookup

The Department of Human Services' Licensing Information Lookup is an online search tool that displays public information for all DHS-licensed programs. 

The information available includes frequently requested public information for licensed programs, including: child care programs, group homes for people with disabilities, and a range of other services for children and vulnerable adults.

Licensing Lookup Features

Licensing Information Lookup (LIL) provides program-specific information about licensed and certified programs. LIL is also a document repository for licensing actions taken by the Department against a program. LIL is searchable by name, city, county, ZIP code, program type, and license number.

What information is available? 

LIL shows the status of the license, the license number, license holder, type of service, initial effective date, what services the program is licensed to provide, any additional information, and the results of monitoring visits. Searches can be done for specific license types and information can be exported into a CSV file.   

Results of monitoring visits for Licensed and Certified Child Care

Summaries of the results of monitoring visits are posted in a chart format for licensed and certified child care centers.  

  • If there are no violations found during a monitoring visit, the monitoring results on LIL will indicate the date of the visit, the type of visit, and that the provider was “in compliance.”
  • If there are violations found during a visit, the outcome summary table will provide:
    • the category of violation;
    • a plain language summary of each violation;
    • an explanation how the violation was corrected (e.g. on site or by submission of documentation). If a violation is appealed, the “How Corrected” column will show “under appeal” for that violation; and
    • the legal citation for the violation

Maltreatment  investigative memorandum and licensing actions excluding correction orders are posted. 

Results of monitoring visits for other Licensed and Certified Services

Maltreatment information, licensing actions and maltreatment investigation documents are posted to Licensing Information Lookup for four years.

Where can I find more information about the way that child care licensing information is displayed?

In 2017 with new federal and state requirements, the department made improvements to the way information was displayed for child care providers. The department developed guidelines for how we display public licensing information for child care programs. The guidelines for posting child care licensing information DHS-7698 (PDF) describe the way public licensing information for child care programs was previously displayed on our website and the changes and improvements we made.  This document is a great resource in understanding what information can be found on LIL and how it is posted.
