How to submit tips

The Minnesota Department of Human Services (DHS) conducts program integrity activities and investigations to ensure taxpayer dollars are used reasonably and appropriately.  We accept tips and referrals from all sources about potential fraud or abuse involving DHS funded programs. 

What is fraud?  Intentional deception or misrepresentation that an individual knows, or should know, to be false, or does not believe to be true, and knowing the deception violates the law or the public trust and could result in unauthorized personal benefit or benefit to others.  

What is abuse?  Engaging in practices that result in unnecessary cost, including providing or receiving services that are not necessary or providing services that do not meet the standards of care.  Abuse is similar to fraud except that there is no requirement to prove or demonstrate that abusive acts were committed knowingly, willfully and intentionally.

How to report suspected fraud or abuse

If you suspect fraud or abuse by someone receiving benefits, health care providers working with the Minnesota Health Care Programs, or childcare providers receiving childcare assistance funds, you can report it to DHS by:  

  • Submitting the Program Integrity Oversight hotline form (recommended)  
  • Calling the Program Integrity Oversight hotline at 651-431-2650 (Toll-free: 1-800-657-3750) 
  • Sending your report via email to OIG.Investigations.DHS@state.mn.us 
  • Sending a letter via US Mail to the Office of Inspector General – Program Integrity and Oversight Division –  PO Box 64982, St. Paul, MN 55164-0982 

What to expect after reporting suspected fraud or abuse

All reports are forwarded to the appropriate entity for review.  Although every report is reviewed, not all reports result in an investigation or action.   

  • Depending on the nature of your report, you may be contacted for more information or follow-up.   
  • Due to the volume of reports received, you should not expect to be contacted about your report.   
  • Please know that investigatory information is non-public pursuant to Minnesota Statute § 13.46, subd. 3, and we are unable to provide updates regarding any particular investigation or complaint until it is resolved. 
  • The final dispositions of fraud/abuse investigations can be requested by submitting a data request.
