Approval criteria
- Patient must be 6 years of age or older AND
- Patient has a diagnosis of attention-deficit hyperactivity disorder (ADHD) AND
- Patient has tried and failed immediate-release AND extended-release methylphenidate AND
- Patient has tried and failed immediate-release AND extended-release amphetamine AND
- Patient has tried and failed immediate-release AND extended-release amphetamine/dextroamphetamine AND
- Patient has tried and failed immediate-release AND extended release dextroamphetamine AND
- Patient has tried and failed immediate-release AND extended release desmethylphenidate AND
- Patient has tried and failed lisdexamfetamine AND
- Patient has tried and failed atomoxetine AND
- Patient has tried and failed extended release clonidine AND
- Patient has tried and failed extended release guanfacine
Quantity limit
5 mg = 170 tablets per 34 days
Additional information
No approvals will be given for the treatment of obesity.
