Therapeutic area - Phosphate binders for chronic kidney disease

Criteria

• Patients who have been stabilized on a combination of Fosrenol and calcium acetate after unsuccessful trials of calcium as a single agent will be issued a PA to continue Fosrenol therapy OR
• Patient has a diagnosis of Stage 3-5 Chronic Kidney Disease (CKD) AND 
  • Has tried calcium acetate consistently
  • Has tried Renagel or Renvela consistently
  • Has not had adequate serum calcium lowering
