Therapeutic area - Antiepileptic

Approval criteria

  • Patient has a diagnosis of Lennox-Gastaut Syndrome AND
  • Patient has tried and failed THREE or more anti-epileptic drugs which include: clonazepam, valproate, lamotrigine or topiramate OR
  • Prescriber is a neurologist and patient has tried and failed at least TWO anti-epileptic drugs which include: clonazepam, valproate, lamotrigine or topiramate

Quantity limits

  • 10 mg tablets: 102 tablets per 34 days
  • 20 mg tablets: 68 tablets
  • Oral suspension (2.5 mg/mL): 600 mL per 34 days

Background information

ONFI (clobazam) is a benzodiazepine indicated for adjunctive treatment of seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age or older.
