Drugs involved

Preferred - No PA required Nonpreferred - PA required
terbinafine itraconazole
Sporanox (branded itraconazole)
Lamisil (branded terbinafine)

Fluconazole is not indicated for onychomycosis and is not addressed in this PA criteria

Appropriate use criteria for Itraconazole

No prior use of terbinafine required

  • Patient has Blastomycosis, pulmonary and extrapulmonary OR
  • Patient has Histoplasmosis, including cavitary pulmonary and disseminated non-meningeal histoplasmosis OR
  • Patient has Aspergillosis, pulmonary and extrapulmonary
