Aging and Adult Services Division (AASD)

AASD supports people to achieve meaningful outcomes. We promote dignity, autonomy and a spirit of well-being for older people, enhancing their quality of life. AASD supports the following types of case management and care coordination:

• Elderly Waiver
• Essential Community Supports
• Relocation Service Coordination Targeted Case Management (RSC-TCM)

Disability Services Division (DSD)

DSD values choice and helping people live where they choose, with appropriate services that assure their health and safety. DSD supports the following types of case management:

• Brain Injury (BI) Waiver
• Community Alternative Care (CAC) Waiver
• Community Access for Disability Inclusion (CADI) Waiver
• Developmental Disabilities (DD) Waiver
