Long-term services and supports rates changes

Service rates and budget changes effective Jan. 1, 2024

The 2023 Minnesota Legislature authorized rate and budget increases for the following services:

Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW)

The following increases apply to AC, ECS and EW:

  • 32.63% increase to AC and EW monthly case mix budget caps
  • 29.61% increase to ECS budget caps
  • Increase to consumer-directed community supports (CDCS) budgets under AC and EW to align with AC/EW case mix budgets
  • 44.78% increase to EW 24-hour customized living daily and monthly limits 
  • Increase to EW customized living daily and monthly limits to 50% of the EW monthly case mix budget caps 
  • 56.67% increase to homemaker 
  • 8.81% increase to home-delivered meals
  • 14.99% increase to caregiver counseling and caregiver training
  •  75.72% increase to chore services, 15 minute
  • 48.21% increase to individual community living supports (ICLS)
  • 170.31% increase to adult companion
  • 28.63% increase to adult day
  • 58.86% increase to respite
  • $600 increase to environmental accessibility adaptations – home and vehicle installation and assessments (to $20,600 from $20,000).

Disability waivers

The following increases apply to Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD) waivers:

  • 8.49% increase to CDCS under BI, CAC, CADI and DD
  • 14.99% increase to home-delivered meals under BI, CAC, CADI and DD
  • 71.4% increase to homemaker under BI, CAC, CADI and DD
  • 14.99% increase to chore services, 15 minute under BI, CAC, CADI and DD.

We will share updates to disability waiver rate system (DWRS) frameworks in October 2023.

Home care nursing and home health services

The following increases apply to home care nursing and home health services:

  • 18.52% increase to home health aide, skilled nursing, OT, PT, RT and ST (14.99% legislative increase plus market basket)
  • 28.84% increase to home care nursing (25% legislative increase plus market basket)
  • 28.84% increase to Consumer Support Grant (CSG) budget(s) based on an HCN home care rating.

The same percentage increases will also apply to the extended home care services.

Personal care assistance (PCA)

PCA rates include changes to the wage component value and implementation factor within the PCA rate framework.

  • 21.43% increase to 1:1 PCA
  • Range of 21.21% to 21.47% increase to shared care and complex PCA
  • 13.24% increase to PCA supervision services
  • 21.43% increase to CSG budget(s) based on a PCA home care rating.

Intermediate care facility for persons with developmental disabilities (ICF/DD)

The following increases apply to ICF/DD:

  • $40 increase to the ICF/DD operating payment rate, excluding the property-related payment rate.  
  • Raise to the ICF/DD operating payment rate floors:
    • Class A facility increase to $275
    • Class B facility increase to $316
  • For each Class A facility that is below the operating payment rate floor after the $40 increase is applied, DHS will make an adjustment to increase the operating payment rate to $275. For each Class B facility that is below the operating payment rate floor after the $40 increase is applied, DHS will make an adjustment to increase the operating payment rate to $316. DHS will make inflationary increases to these rate floors in future years.
