Approval criteria

  • Patient has diagnosis of onychomycosis AND
  • Patient has a letter of medical necessity stating why generic itraconazole capsules cannot be used

34 tablets per 34 day supply

Background information

ONMEL, an azole antifungal, is indicated for the treatment of onychomycosis of the toenail caused by Trichophyton rubrum or T. mentagrophytes. The recommended dose is 200 mg once daily for 12 consecutive weeks. The equivalent dose of Onmel can be achieved using 2 capsules of generic itraconazole 100mg capsules.
