Health plan member services phone numbers

While all health plans provide the same benefits covered by the program you are enrolled in, they have different networks of providers who can provide those services. You can check the provider directories in the table to see whether your provider works with a particular health plan. 

If you do not see your provider listed, please contact the health plan to check whether the provider is in the health plan's network. The directories linked in the table may not list certain specialty providers. Contact the health plan to ask about whether certain specialty providers are in the health plan's network. 

You can change your primary care provider or clinic anytime by calling your health plan member services. The phone number is on the back of your health plan member ID card. It is also available through the health plan member services phone numbers link on this page.

The table provides links to each health plan’s 2024 provider directories. Some health plans use the same directory for 2024 as 2023, so only one directory is posted.
