Section 811 Project Rent Assistance Application DHS-7102 (PDF) is used by transition coordinators for the HUD Section 8 Project Based Rental Assistance Program. It offers subsidized rental units in designated properties for persons under 62 who are enrolled in Moving Home Minnesota or who are experiencing long-term homelessness and serious mental illness and are receiving PATH services.

Interim assistance agreements
- SSI Interim Assistance Authorization DHS-1795 (PDF) compels the signer to agree to repay funds received from General Assistance and/or Group Residential Housing if the applicant is later receives funds from Supplemental Security Income.
- Interim Assistance Agreement DHS-1795A (PDF) specifies conditions in which the applicant agrees to repay money from General Assistance and other sources if later determined to be eligible for other state or county funds.
