Culture of Safety critical incident review

The Disability Services Division’s culture of safety initiative is a proactive strategy to improve outcomes within disability services. The culture of safety initiative, which follows the collaborative safety framework, focuses the response to critical incidents on learning and engaging direct care workers.

The culture of safety initiative uses a systemic critical incident review team to study critical incidents, as required by state law. See Minn. Stat. §256.01, subd. 12b.

Note: The culture of safety systemic critical incident review does not replace the process for screening or investigating cases of alleged maltreatment of an adult under Minn. Stat., §626.557.

If you have questions about DSD’s culture of safety initiative, incident review model or trainings, email DHS.
