    FDA approved indication

    Calcitriol ointment is indicated for the topical treatment of mild to moderate plaque psoriasis in adults 18 years of age and older.

    Criteria

    • Patient has mild to moderate plaque psoriasis and is 18 years of age and older AND
    • Patient has used Dovonex (calcipotriene) cream consistently for at least 8 weeks without success
