Therapeutic area - Hypoparathyroidism
Approval criteria
The patient must meet all of the following criteria:
- Is 18 years of age or older
- Has a diagnosis of hypocalcemia secondary to hypoparathyroidism
- Has tried and failed calcium supplementation with vitamin D
- Has sufficient 25-hydroxyvitamin D stores (chart documentation must be provided at time of request)
- Serum calcium is above 7.5 mg/dL prior to starting Natpara (chart documentation must be provided at time of request)
- Will be concurrently taking a calcium supplement and vitamin D
Quantity limit
Two medication cartridges per 28 days
