The Disability Services Division (DSD) recognizes that requests for assistance are received through multiple channels. To ensure our overall communication, training, and technical assistance is provided consistently, we are implementing a single web form to provide a consistent method for people to request assistance from DSD.

This form can be used by anyone who needs assistance from the Disability Services Division. I.e. – how to receive services, how to become a PCA, having service issues, etc.

The DSD contact form is available on this webpage and the Community-Based Services Manual (CBSM) homepage. The form link replaced some DSD policy area email addresses on DHS and CBSM webpages.

The following DSD policy area e-mail addresses will not be replaced with the form link at this time:

Yes, you may still send emails to DSD policy area email addresses. Emails sent to any DSD policy area email addresses will still be received and an autoreply message will ask you to use the DSD contact form for future inquiries.

Yes, the information submitted through the form is secure and will only be available to the Disability Services Division to further assist you.

Once you submit an inquiry, you will receive a confirmation email from the Disability Services Division within 5 minutes.

You can reply to the confirmation email that you received from the Disability Services Division to check on the status of your inquiry or follow up with additional information.

On the form, under each role you can select the option “Other questions or concerns not listed here” to submit your specific question or issue.

For urgent issues, we recommend you use the online form. This will immediately route your inquiry to the correct DSD policy team, which will reduce the amount of back-and-forth necessary to complete your request.

Yes. All DSD eDoc forms will remain the same and may be accessed wherever it is listed (CBSM, webpages, etc.).

Use the contact form to reach your RRS or the appropriate policy team to assist you further. The form will also help DSD to quickly identify technical assistance trends and help us develop strategies to address technical assistance and training needs for lead agencies.

No, this will not delay the response time. Your inquiry will be routed to the correct DSD policy team immediately, reducing the amount of back and forth necessary to complete a request.

National provider identifiers (NPIs) are the standard unique identifiers to use in submitting and processing health care claims and other transactions. If you are a provider eligible for an NPI, you must obtain your NPI number(s) from the National Plan and Provider Enumeration System (NPPES) before you enroll with Minnesota Health Care Programs (MHCP).

The following providers are not required to obtain an NPI, but may have the option of using an NPI registered to the provider:

• Home and community-based services
• Personal care provider organizations
• Day training and habilitation
