Approval criteria

• Patient must be 10 years old or older AND
• Patient has had a skin test positive to any of the 5 grass species in the product AND
• Patient has tried and failed at least one drug out of 2 of the 3 following classes in the last 730 days: intranasal corticosteroids, oral antihistamines, leukotriene receptor antagonists OR
• Patient has tried allergen-specific immunotherapy injections in the last 730 days

Quantity limit

300 IR: 34 tablets per 34 days
