Description

Doryx® Delayed-Release Tablets contain doxycycline hyclate for once daily dosing. Average prescription cost is $180 for 30 tablets.

Criteria

• Doryx is being prescribed as adjunctive treatment for severe acne AND
• Patient has tried but is unable to tolerate generic formulations of doxycycline

Contraindicated for use during pregnancy. Pregnancy Category D.
