            Cetraxal (ciprofloxacin) 0.2 %

Criteria

Treagan (acetic acid, antipyrine, benzocaine)

• Patient is being treated for acute otitis media AND

• Ofloxacin 0.3% otic (Foxin® Otic) was recently used but was not effective  

Cetraxal (ciprofloxacin) 0.2 %

• Patient is being treated for acute otitis externa AND
• Ofloxacin 0.3% otic (Foxin® Otic) was recently used but was not effective
