Criteria

• Patient has tried at least two preferred single agent triptans, of which one trial must be sumatriptan (Imitrex), but receives inadequate relief from single agent triptan therapy AND
• The patient can not physically take a triptan an NSAID together

Quantity limit

One package of 9 tablets allowed per month.
